When he phones her, she tells him to stop calling, but he acted as if he had not understood her.

A. she tells him to stop calling, but he acted as if he had not understood her Looks correct[/b
B. she [strike]told[/strike] him to stop calling, but he acted as if he had not understood her
C. she tells him to stop calling, but he acted as if he did not [strike]understood[/strike] her
D. she tells him to stop calling, but he acted as if he has not understood her [b] maybe
E. she tells him to stop calling, but he acted as if he does not [strike]understood[/strike]her

Issue is the use of present/past participle confused :(

I go with D. OA?

Boils down to C and D because everything should be in present tense.
In C, "he did not understood" is wrong.

Has understood - use of present perfect is needed to keep the whole sentence in the same tense form
Now it makes sense :)
