A. she tells him to stop calling, but he acted as if he had not understood her
B. she told him to stop calling, but he acted as if he had not understood her
C. she tells him to stop calling, but he acts as if he did not understood her
D. she tells him to stop calling, but he acts as if he has not understood her
E. she tells him to stop calling, but he acted as if he does not understood her

I go with D. OA?

Boils down to C and D because everything should be in present tense.
In C, "he did not understood" is wrong.

she tells him to stop calling, but he acts as if he has not understood her

Has understood - use of present perfect is needed to keep the whole sentence in the same tense form
Now it makes sense :)
