Hello. I'm a beginner in the MBA application process myself, but my educated guess is that a business school interview is not unlike a job interview. That means that you will probably do well to dress in semi-formal to formal attire: slacks, dress shoes, button down shirt, and tie, at least. In fact, most men I saw at the Los Angeles QS MBA World Tour this weekend were in jackets as well.
As for the interview questions, the most common theme I've been seeing in my own research starts at the top level and works its way down: Why are you pursuing an MBA? Why now? And why this particular school?
It's unclear from your post, but have you completed the entire application to the University of Wisconsin-Madison's MBA program and just haven't done the GMAT yet? If you haven't, then download the application and review the essay questions--I've heard that sometimes the adcom will revisit those with you even though they have your application. Also, because you come from a military background, I would be willing to bet that they will want to explore this and how it uniquely qualifies you for business school admission--especially as it relates to your leadership abilities.
