Official Explanation:

A. "unsure about" is idiomatic. The construction is precise.
B. "unsure with respect to" is unidiomatic.
C. "unsure regarding" is unidiomatic. "should" is not the correct usage; it alters the meaning.
D. "was" is necessary before "unsure". "in regard to" is unidiomatic.
E. "lacked certainty in regard to" and "suspicion regarding the..." are wordy and awkward.
