Let the grade in the fourth test be x

$$\frac{86*3 + x}{4} = 88$$ --> x = $$88*4 - 86*3 = 352 - 258 = 94$$

Therefore, Susan scored a 94(Option A) in her next test.

Alternate approach
In the first three tests, she scores $$2*3 = 6$$ less than the average.

Therefore, Susan would have scored $$88 + 6 = 94$$ in her last test!

New required average-Old avg= 88-86=2
This number 2 has to be added to all four test scores to get the required avg 2*4=8
required test score = 86+8=94.

in three exams susan has average of 86.
total: 86*3=258
he want to up his average to 88 in 4 exams.
total: 4*88=352
difference= 352-258=94
thus he has to secure 94 in 4th exam in order to keep his average grade 88 in total 4 exams.

IMO A
total of 3 tests= 3*86=258
258+x/4=88 (where x is new score in next exam)
88*4-258=x
352-258=x
94=x
