To answer your query, Option D is definitely incorrect, as it incorrectly uses "Based on a recent study by the National Institute of Health" to modify "teen-aged boys and girls", illogically implying that the boys and girls were based on a particular study; the intended meaning seems to be that on the basis of this study, a conclusion was reached about these boys and girls; remember, in the “phrase + comma + noun” and “noun + comma + phrase” constructions, the phrase must correctly modify the noun; this is one of the most frequently tested concepts on GMAT sentence correction.
