In quant, I think the basic concepts of all relevant areas indicated in OG, shall be done. After this, practice of questions is sufficient. Usually questions of OG are good. You can take some questions from other sources too. I feel, there is no need to give specific stress on particular area. If you feel difficulty in any particular area, you can devote some additional time on that.
Level of difficulty is usually same in gmatprep. It seems a bit difficult on actual exam, but that may be due to examination pressure also.
Last few questions could be difficult in OG13, but you should not worry about that. I did not do OG13 actually, I was having OG12. If you are finding some difficult questions, just try to learn any new concepts or question type.

There could be few such questions, but CR practice from OG is good to make one comfortable with CR question types. Along with that, concentrating more on logical problem solving and being open to new question types could be helpful.
