A)

the first action (had realized) should indicate that it happened before the second action (have gone). first action must be past perfect and the second present perfect.

the first sentence is the conditional clause and the second is the main clause. normally we would use "if" in the conditional clause but sometimes it can be replaced by a subject-verb inversion.

option A - Had I realized, I would not have
This is conditional 3 - past perfect in the condition and I would have in the result.
