Nothing wrong with the Original Sentence, errors in other options highlighted in red, further adapted to is correct idiomatic usage, hence Correct usage must be (A)

No. The pronoun "that" cannot stand for a clause.

Option A would be correct if it stated, "Human beings are not well adapted to change and that fact (or that human characteristic) creates many problems in the workplace because change has become the only real constant in companies today."

In the rewritten sentence, that in that fact or in that human characteristic is a demonstrative pronoun.
That points to fact or to human characteristic, both of which are summative modifiers (a word or phrase that we insert to describe an idea in the sentence, often to add emphasis or because we need a simpler subject that is not a clause).
The name of the inserted words is not important.
Just know that we can make up a word or phrase that summarizes an idea or clause in order to avoid convoluted sentences.



*EDIT - thanks Skywalker18: The source of this question says Manhattan GMAT. I doubt it. If it's from Manhattan anything, I'd bet it's from Manhattan Review.
Manhattan GMAT and Manhattan Review are not the same. Manhattan Review puts out questions whose quality is uneven at best.
If the question is from the renowned Manhattan Prep, the source will say MGMAT.

I think you mean to say that the source of this question is Manhattan Review. Recently, I have come across many such sub-standard questions, tagged as Manhattan GMAT, whereas they are most likely from Manhattan Review.

"adapted for change" is wrong --> eliminate B, C, and E
D has past tense -> created -> but this human tendency still causes trouble, so past tense changes the meaning. Eliminate D.

Why A alone?. Which choice for that matter is using that dangling demonstrative pronoun correctly? But the author seems to be blissfully oblivious of that rule. Therefore, why do you alone bother? You will do well to go to the next question sooner than later.
