Check out this post by BB where he has shared some of the best error logs made by GMATClub members. One of them will surely fit your requirements.

Hi Himanshi24sharma,

If you're looking to keep track of only a few things (as you should), you could easily set your own error log up in a google/excel sheet: source/question number/correct-incorrect/time taken/comments.
