Hello,
faycalshb. I will address each question you posed one by one, but first, I want to say that I agree with what
DisciplinedPrep wrote already, and I think the name says it all, really. To hit a target score, you are going to have to put in the time and effort to achieve such a result. Speaking specifically to the point made about tracking down explanations to official practice questions, you can find them one after another by typing out the first few words of the question stem as a search, or also by using a few keywords. That is typically good enough to bring up the exact question in the first few search results.

How can I achieve these scores with such low accuracy in both Q & V ?
Your score is based way more on the level of difficulty of the questions you are missing and answering correctly than on the strict number of questions you are missing. I remember, too, missing way more Quant questions than I was accustomed to the first time I laid eyes on an official practice test, but the section score came out fine. That is just how the test is calibrated to find your ceiling in Verbal and Quantitative measures.

From your opinions how should i continue my prep to achieve a 680+ on the real exam?
It sounds as though you are starting with high-quality preparation materials, so as long as you continue to learn strategy from resources you find useful and combine that knowledge with practice through official questions--the
OG is the single best source out there--you should have no trouble hitting your mark. It just depends on how much time you have and how diligently you are able to practice. Some people like to crank everything out in a month, while others feel more at ease with a study plan that stretches across several months.
Best of luck to you in your studies.
