How do you ask your manager for a letter of recommendation?

Hi all,
So I'm in a bit of an awkward situation and I'm sure there are many others who fall into this category but I haven't seen any postings/threads about it so far.
I started a new job in January 2011, and I'm planning on applying for MBA schools in Round 1 in 2012 (so around September/October 2012) for 2013 intake.
By September/October 2012 I'll have worked at the company for about 1 yr and 9-or-10 mths or so. Here are my concerns:
1) Is that a long enough time period to be able to get a good reference letter from my current manager?
2) Since I'll be applying 1 full year before I actually start school, will it be awkward since I'm essentially telling my manager that "I'll be leaving the company a year from now, please write me a recommendation letter." - or what's the best way to go about doing this? I was thinking of applying in Round 2 instead to shrink that time gap a little bit, but I've heard that Round 1 is ideal.
Any insight would be greatly appreciated. Thanks!
