Is parallelism alright in option B? melting of polar ice caps is a prepositional phrase and the rising sea level is a participle phrase. Am I right? IanStewart

No, B is not parallel. It would be if it said "consequences include melting ice caps and rising sea levels", or if it used prepositions in both, say "include the melting of ice caps and the rising of sea levels" (though "the rising of" is really awkward English). The OA is a variation on the latter construction that reads more naturally.

To answer your query, the fact that "rise" is preceded by an article - "a" in this case - is a clear indicator that it is a noun rather than a verb; remember, articles are only used with nouns.
