In short, I think you have a competitive profile for most (if not all) of the schools you listed here. I do wish your GMAT score were a BIT higher, but I think you can get into at least one of these schools with the score and the profile you have now. Your medical degree, your work experience, and your international exposure will all be great assets for you.
Doctors are rare in business school, but that's an advantage for you as long as you can demonstrate that you "get" what business school is about, have good reasons for wanting to go, and have realistic post-MBA goals. You seem to meet these criteria, so that's why I feel optimistic about your chances here. The key now will be to write great essays and great great letters of recommendation that highlight all of the traits that MBA admissions officers look for in applicants: leadership, teamwork, and maturity.
It's hard to rank your schools in terms of probability of success other than to tell you to start at the lowest-ranked one and work your way up the list, going from easiest (to get into) to hardest.
But I do think that you can aim as high at Wharton/Stanford/Kellogg/Booth... Again, the key will simply be how strong your application is at this point. And you may be surprised... You could get into Wharton but not get into UCLA, when you expected the opposite to happen. That's why we advise our clients to apply to 4-5 schools if they can... There's enough random chance in the admissions process that you just never know.
