Doesn't the word "them" refer to something plural in C ?
Whereas the "cochlear plant" is singular. Isn't it ?
Why is it not E ?

"them" refers to "signals", which is plural.

You can also think about the meaning. Implants are not sent to the brain, signals are. The pronoun should logically refer to signals.

About E:

I took notes from a correct SC answer that looked like this: “She was elected to be the second woman in the state to serve on the state bench”

In this case "to be" was correct because it emphasized the purpose. But wouldn't this conflict with your explanation because "she" isn't the agent of the action. She doesn't elect someone, but she gets elected.

Can you please explain?

A is very repetitive, this suggests that it is not a good choice. B is wrong because it implies that the electric signals are implanted into the brain. C seems correct. D again messes up the order of words, ‘implanted directly…’ modifies ‘auditory nerve’, implying that the nerve is implanted. E is correct but ‘to be recognized’ is a little awkward and wordy. C is the better option.
