Honestly just went with how I’ve heard the phrase “plead guilty” in myriad articles etc. You always plead guilty “to” something. A,B and C gone.

Between D and E, the “something” has to be an action. In D, “it’s failure” is a clause whereas in E “failing to report” is an action phrase.

Went with E. Waiting for OE.

All options with the pronoun "its" are incorrect since its can refer to either the bank or the federal district Court. So B, C, D are out. Option E uses infinitive "To failing" which is never correct, so it leaves us with option A.

"A" seems correct. In rest its is ambiguous.

It is very tempting to choose (A) in this question, but, this is an idiom problem, and the correct idiom is plead guilty to, thus, (A) - (C) is out. Between (D) and (E), (D) uses incorrect "its". So, (D) is out. Final answer left with (E).
