Because the clause "is concerned primarily......................" is a report, thus "that" is necessary. Down to A & D.
A is wrong because it compares "the nobility of old" and "historians"
D is the best.

Confusion should be between A and D.

A is incorrect because nobility of old is compared to historians

D is correct because royalty is compared logically to nobility.

A. Much like the nobility of old, historians maintain that today's royalty wrong comparison.

B. Today's royalty, much like the nobility of old, it is maintained by historians, 'it' is redundantly used.

C. Much like the nobility of old, historians maintain today's royalty wrong comparison.

D.Historians maintain that today's royalty, much like the nobility of old, Correct answer choice.

E.Historians maintain, today's royalty, much like the nobility of old, redundnat useage of the comma,as it is not required after the maintain.

Compare the correct thing. Can't compare royalty to historians or something else. Cancel the awkward sentences and you are left with D
