First of all, check the instructions from the school. Some will want new recs and some won't. Follow the instructions. If you have discretion, then yes at least one new rec focused on recent achievements would reinforce the message of growth and change since you last applied.

Different people? Again that depends. If you have a new boss, then yes. If you have the same boss, but have assumed different duties, then no.
