Doubt: Although there is a meaning difference in option A and B. How do we recon which is the intended meaning of the question. Since in my opinion both options make sense.
Meaning option A - The discrepancy was already known and the reason for the same was being investigated. And thus, it was found that the records had been tampered.
Meaning option B- The discrepancy was NOT known and during the inspection of the records (usual practice to check records) a discrepancy was found.

Just a commentary on the second half of the sentence:
"...he found that the files had been tampered with."
Note "found" is past tense and the past-past tense "had been tampered with" puts extra emphasis on the fact that first the records were tampered with---THEN he found the records.
Technically, "he found that the files were tampered with" could also work- but it would not have as much of an emphasis on the distinction in time.
Of course, this is the non-underlined portion of the sentence so it does not affect the answer, but just a note to point out.
