Prohibit from is correct idiom
only seen here in option B ;

Meaning: Washington passed a law that stops public facilities that allowed smoking 25 feet within its premises
A: Prohibit from is correct usage. All other usage are incorrect
B: Correct
C: Same as A
D: Banning by public facilities. Complete meaning distortion. Also the law never mandated for smoking 25 feet within premises
E: 180 degree opposite meaning. Instead of banning this permits Also the law never mandated for smoking 25 feet within premises
Mandate for is correct usage or not? Well I don't know so if confused don't break your head over it

eliminating
E/D- use of which as restricting pronoun isn't appropriate here
A- "prohibit that PF allow" would have been the correct subjunctive structure
Between-B and C prohibiting to allow is an unidiomatic usage.

so B is the winner

the meaning is that the law prohibits places from allowing outside smoking within 25 feets of their gates, windows and vents.
Prohibit from is idiomatic.
