MANHATTAN REVIEW OFFICIAL EXPLANATION:



Once again you have to figure out whether to use the simple past or the present perfect. The action has a consequence in the present so the present perfect should be used. Eliminate B, C and E. In addition, did the smoking damage her lungs only once or over a period of time? Clearly over a period of time. That eliminates choices B, C and E right away. Now you have to determine whether serious modifies an adjective or noun. It refers to damage, so it should be left as an adjective. Choice D is the correct answer.

Breakdown to simple parallel structure,


Annabelle’s pack-a-day smoking habit has done seriously damage to her lungs. : This is not correct since seriously is not the correct usage,
Annabelle’s pack-a-day smoking habit has done potentially fatal damage to her lungs. : This is correct

So only D correct the first mistake and retains the parallel structure.

Adverb---->Modifies---->Adjective------> Noun { Very much grammatical, As per MGMAT SC 13}

Annabelle’s pack-a-day smoking habit has done serious and potentially fatal damage to her lungs.

1. Serious and Fatal are adjectives ----- Must be parallel.
2. Potential ( Adverb ) modifies Fatal (Adjective ) which in turn modifies the word " Lungs " (Noun)

The sentence is of the structure : Adverb + Adjective + Noun in which the adverb modifies the adjective which in turn modifies the Noun...

in my opinion adjective 'fatal' modifies the noun 'damage' not 'lungs'.
correct me if i am wrong.

Here D is the correct answer . The parallel markers X and Y are used correctly , also the use of 'has' is correct.

1) This question is a classic case of parallelism. The question is trying to trick by trying to show "seriously" and "potentially" as the things that are parallel. But the actual parallelism is between "seriously" and "fatal" this eliminates A & B.
2)The underlined part of the sentence should end with "to" for the sentence to make sense this eliminates C & E.

So the answer is D
