The question is testing three elements - Parallel construction, Pronoun reference and Meaning.

The Massachusetts Department of Labor Relations requires the state’s over one hundred thousand private employers to display its union rights sign and informs employees of their right to join or form a union or to take collective action to improve its working conditions.

Meaning: The Department gave an instruction to Private employees - Display the union rights of the company. Now the union rights highlight the various rights the employees have. With this meaning Lets do POE

>> 1. Meaning wise, the choice states that the department informs the employees of their rights - Incorrect as the intended meaning is different.

Informs is used as a verb for the subject department as if department informs employees. So the verb 'informs' need not be parallel to the verb 'requires'. This type of error is termed as superficial parallelism. Logically speaking department requires the state to display its union rights sign. Remember union rights sign is used as a compound noun here. Moreover 'its' is a singular possessive pronoun. 'its' cannot refer back to a plural noun 'employees'
B)sign, which informs employees of their right to join or form a union or to take collective action to improve their
The relative clause that begins with 'which' modifies the noun 'sign'. Subject of the verb 'informs' is 'sign'. Moreover the pronoun 'their' refers back to the noun employees. No error so correct answer choice.
C)sign, informing employees of its right to join or form a union or to take collective action to improve its
'informing' is used as a present participle with comma. We use present participle with comma when we have a cause and effect relationship in the sentence. Here the sentence doesn't convey a cause and effect relationship. Moreover a singular possessive pronoun its cannot refer back to a plural noun 'employees'. So this is also wrong.
D)sign and inform employees of its right to join, form a union, take collective action or improve its
like the option A 'inform' is used as a verb and 'its' is a singular possessive pronoun. So this is also wrong
E)sign, which informs employees of their right to join, form a union, take collective action, or improve their
sign gives employees two option. Either join or form a union or to take collective action. We need an infinitive. So it should be to improve. So this option is wrong
