1st split:
Choppy and askew/genuine are a kind of opposites so yet is more appropriate here. We are left with A and D.
The play (singular) creates. D is eliminated, A is the winner.

My take on the use of 'each' here:
Each is definitely singular when used as a subject: Each of them is....
However, in A the subject is 'the play’s three separate acts' (plural). Therefore, it's ok to say: three acts create...
E is here something like modifier meaning ' every thing, person, etc. in a group of two or more, considered separately'

A. Choppy and askew, yet genuine, the play’s three separate acts each create- Correct.
Choppy and askew are opposite of genuine so the correct conjunction is used. The subject of the sentence is the play’s three separate acts, hence create is correct. Please note singular verb form is required if each/every is the subject of the sentence or the subject of the verb is preceded by each/every.

B. Choppy, askew, and genuine, the play’s three separate acts each creates- Incorrect.
This choice changes the intended meaning of the sentence as genuine is meant to be contradictory to choppy and askew. Plural verb create is required for the subject ‘play’s three separate acts’

C. Choppy, askew, and genuine, the play has three separate acts, each of which create- Incorrect.
This choice changes the intended meaning of the sentence as genuine is meant to be contradictory to choppy and askew. Plural verb create is required for the subject ‘play’s three separate acts’. Further this choice is a bit wordy.

D. Choppy and askew, yet genuine, the play, which has three separate acts, create- Incorrect.
Although uses the correct conjunction, the plural verb create does not agree with the subject the play.

E. Choppy, askew, and genuine, the play’s three separate acts creates- Incorrect.
This choice changes the intended meaning of the sentence as genuine is meant to be contradictory to choppy and askew.

Official solution
As you assess your Decision Points on this problem, recognize that the subject-verb agreement differences ("create" vs. "creates") on the right-hand side of the answer choices belong to a much more concrete and recurring error type than the idiomatic structures on the left. Avoid the temptation to fix the first difference you see (how to group "choppy," "askew," and "genuine") and focus on the decisions that you've made (or will make) hundreds of times in practice and will make several times on the GMAT!

For choices A, B, and E the subject of the verb "create(s)" is "acts," begging for a plural form of the verb, "create." Only choice A supplies that form, so you can eliminate B and E.

Choice C changes that by adding the verb "has" to "the play" - making "the play" the subject of the first clause in the sentence - and setting up a subordinate clause "each of which create" for which "each" is the subject. Alone, "each" is a singular subject and therefore would require the singular form "creates." Because that is not the verb listed in choice C, choice C is incorrect.

Choice D also makes a change to the structure, placing "three separate acts" within a modifier and therefore making "the play" the subject of the verb "create(s)." Because "the play" is singular, and "create" is plural, choice D is also incorrect.

B. Choppy, askew, and genuine, the play’s three separate acts each creates
C. Choppy, askew, and genuine, the play has three separate acts, each of which create
D. Choppy and askew, yet genuine, the play, which has three separate acts, create
E. Choppy, askew, and genuine, the play’s three separate acts creates

If we try to identify the subject in the answer choices, we have only two choices:

1. the play’s three separate acts
2. the play

In first look we may opt to eliminate the choices with "1. the play’s three separate acts" subject, because of possessive form "the play’s". But choices C and D(subject 2) do not use correct subject verb agreement.

C. Choppy, askew, and genuine, the play has three separate acts, each of which create
it should be each of which creates.

D. Choppy and askew, yet genuine, the play, which has three separate acts, create
D. Choppy and askew, yet genuine, the play, which has three separate acts, creates

A. Choppy and askew, yet genuine, the play’s three separate acts each create
Correct choice as it uses the correct subject verb agreement and contrast.

Concept:

1) Each of the three trucks GOES.
2) Three truck each GO.

The above question makes the use of the above concept. Both the sentences written above are grammatically correct.

The question is an example of sentence #2 written above. We need a plural verb.

B and E can be eliminated.

C - each of which... from sentence #1 shown above, it must be followed by a singular verb. But in C, the verb is plural. Eliminate C.

D - the play is used as subject but the verb that follows the subject is given as plural. Eliminate D.

A is correct.

Answer A
Eliminate B, C, E because they change the meaning. Choppy and askew are in a separate grouping from genuine.
D. "the play" ... "create" is wrong

A. Choppy and askew, yet genuine, the play’s three separate acts each create Subject and the verb agree!
B. Choppy, askew, and genuine, the play’s three separate acts each creates Since the subject (acts) is plural, the correct verb will be create.
D. Choppy and askew, yet genuine, the play, which has three separate acts, create Change in meaning. Also the subject (play) doesn't agree with the verb creates.
E. Choppy, askew, and genuine, the play’s three separate acts creates should be acts create.

Choppy and askew, yet genuine, the play’s three separate acts each create a unique perspective on the characters’ view of success and fulfillment.

Use of Each

To Solve it, we need to be clear about the use of the word "Each".

1)- When used In front of Subject (Prefix) :-

Whenever Each is used infront of the subject of a sentence, it make the subject Singular regardless of whether the subject is singular or Plural.

For example :-
Each of the students is going on a trip.
Each of the members of the club gets a free ticket to the concert.

2)- When used after the Subject (suffix) :-

When each is used after the subject, it doesn't change the nature of the subject i.e. The singular or plural nature of the subject depends entirely on the subject.

For example :-
The four largest ship fleets each account for at least 15 percent of the budget.
Jack and Jill each want a chocolate.

Now, On the basis of the above mentioned rule, we know that the verb of the Sentence must be create(plural) because the subject is acts(plural).

Based on this Choices B & E can be eliminated because they use a singular verb Creates.

Choice C can be eliminated using the same rule. It now uses Each as a prefix(of which = acts) and then it uses a plural verb create. We know by now that using Each as a prefix changes the verb to a singular verb.

Choice D makes "the play" (Singular) subject of the sentence and then uses a plural verb create. So it can be eliminated as well.

We now have a winner, The correct answer is choice A.
