learningyear Yes, C is incorrect for multiple reasons.

You're right that we normally wouldn't use a comma to connect 2 parallel verbs with "and"; generally, there would only be a comma to connect 2 independent clauses, including a new subject.

For example:
Joe runs and jumps.
Joe runs, and Bill jumps.

The bigger issue in C is that the meaning is illogical: "A tiger escaped ... and caused ...". The tiger itself is not the cause; rather, the cause is the full clause (the situation, including the action of escaping): "A tiger escaped his enclosure..."

So, E is actually correct. This is an ideal use case for this "comma verb-ing modifier", which is often used to connect cause and effect: "A tiger escaped..., causing some to question..."

Additionally, A and B are wrong for the use of "it". Presumably, "it" refers to the tiger, and just as we saw in C, the tiger itself is not the cause.
