My choice is A for first question for the following reasons:
- Modifier must touch the noun it modifies, in this case the last phrase must modify "inadequate treatment"(noun).
- If modifying phrase uses "Having+ V3" that means modifying phrase refers to a time before that of the main clause. Here inadequate treatment is only possible before the action of too low ---- etc. takes place.

My answer for second question is A
Where refers to homes.
