I gave an official GMAT mock today and got a score of 83 in DI with one incorrect answer which was Q no. 18. I did not change any of my answers in review.
Is it possible for something like this to happen in the actual test day?

The scoring of the official practice tests is supposedly the same as the scoring of the actual GMAT. So, it could be that the answer to your question is yes.
That said, was that practice test a retake? If so, it could be that the test ran out of hard questions and presented you with a relatively easy question with the result that, when you missed it, you missed a relatively easy question, driving down your score substantially.
In any case, that outcome is definitely pretty extreme.
A good thing is that such outcomes are not common. So, you won't likely get such a result when you take the GMAT.
