difference between D and A is:
usage of 'was' in second element
and usage of 'had' in 3rd element
A can be read like:
She had become too ill to serve in 1983 so she was discharged.
so Option A is the winner here.

"Sampson was too ill to serve" action happened before "she was dischared in 1873". As the latter action is in past tense, the former action should use past perfect tense.
A & B are the only options.
B distorts the meaning of the sentence by inserting "while"
Thus A is the only choice that eliminates above errors and is the best choice.
