how is the placement of 'all' change the meaning of the sentence?

We need "such as" here so eliminate A,B

Comparison is between countries, so eliminae C

Eliminate D because of the placement of "All". In D this seems to be saying the fertility rates (of which there are several) exceed all the fertility rates of its neighbours (which implies that each neighbour could have many rates). This does not make sense

In E, the comparison is clear. It correctly says that the rates of the Phillipines > rates of ALL neighbours individually.

First, you narrow it down to CDE, because you want to use the phrase "such as". 
Second, the subject in the sentence is "the phil nation" not "the rate", thus only E is correct.

Philippine nation rates exceed  all those  of  Southe Asian Countries --> all rates of South Asian Countries ( here it looks like all rates of South asian countries together -- which is wrong)
Philippine nation rates exceed   those  of all  Southe Asian Countries (Here it exceeds indiviually each South Aisan neighbours' rates)
