how is the placement of 'all' change the meaning of the sentence?

Philippine nation rates exceed all those[rates] of Southe Asian Countries --> all rates of South Asian Countries ( here it looks like all rates of South asian countries together -- which is wrong)
Philippine nation rates exceed those[rates] of all Southe Asian Countries (Here it exceeds indiviually each South Aisan neighbours' rates)

Such as vs. Like usage issue – Such as is used to provide examples where as like is used to provide similarity.

Eliminate A, B

C. such as Thailand and Indonesia, the fertility rates exhibited by the Philippine nation are consistently exceeding those of all [ Passive Voice – eliminate it]

D. such as Thailand and Indonesia, the Philippine nation's fertility rates consistently exceed all those of [ Illogical comparison – nations’ fertility rates compared against other nations – eliminate it]

E. such as Thailand and Indonesia, the Philippine nation consistently exhibits fertility rates that exceed those of all
