Hmm.. this is a tough one for me. We can easily eliminate C, D and E due to their awkward construction.

So it is between A and B. Now, B uses "known to have recovered" while A says "known to recover"

I feel B is better because its present perfect tense agrees better with the rest of the sentence.

I'll go with B

I'll go along with the majority - B is correct

C,D and E and wrong because they are either unnecessarily wordy or are grammatically incorrect.

I am going with B, because - few people are known to have recovered, maintains the tense, and is parallel - t have recovered... have appeared

A should be it.

Choice is between A and B

B says that people have recovered from the decease and they dont anymore.

B has more problems. look at its construction
few people are known to have recovered from the disease (They are done recovering) once the clinical symptoms have appeared (have appeared, shouldn't it be just "appeard" since they have already recovered.

A says, they still do.

It can not be B.

I too think it should be A.

in B the second verb should be in a past perfect tense, "had appeared".
