The question tests S-V concept. Rate is singular therefore verb should be has. State Welfare Organization is also singular therefore verb has should be used.
A & B can be straight eliminated due to use of plural verb have for rate. C can also be eliminated due to use of plural verb have for State Welfare Organization.
D correctly uses singular verb has for both subjects. E can be eliminated for distorting the meaning of the sentence and for being awkward.

We need first HAS for " rate of poverty, " which is singular . Rate of something is considered as singular.
Only option C and D are left .
Second HAS singular we need for Organization "state welfare organization" which is also singular subject.
