I would Go with A...
Because Having is used for things already done and then a past tense verb is used....

But usually on GMAT having is wrong.
Someone Correct me and please give a better explanation.

Providing my insight to the question:

This comes to a case of meaning.

A. Having always confessed distaste of such habits. The writer claims that Dr B possesses the distaste of such "habits" and continues to smoke, which is a contradiction in of itself.

B. For all his confessed distaste of such habits The writer claims that although Dr. B has distaste in such "habits", he continues to smoke. This answer is a better choice because it acknowledges the contradiction between the two phrases.

A) "having always confessed distaste of such habits", means that he (Dr B) completely doesn't like such habits. Saying this along with 'began smoking' is not valid.

B) 'For all' in other words can be said as " in spite of " which makes complete sense when put into the sentence. correct
