Hi Andrew,
I contacted the proctor twice and did not get a helpful reply i.e We cannot help you directly with a question. How should I report this to GMAC and how should I present evidence?

Just finished the Online GMAT. When I reached questions requiring me to answer questions about bold-faced texts, I could not see any text bolded in the passage in the system. Was I supposed to do something to activate the bold-face? I contacted the proctor twice and did not get a helpful reply i.e We cannot help you directly with a question.
Add something about the number of boldface questions you believe you had encountered, and express concern about the potential outcome for your test. I think that will just about do the trick. Just do
not sit on this. The longer you wait, the less likely you will be to see a positive outcome, in my view.
