I have given my GMAT twice with pathetic scores. I am planning to take it one more time and get a better score. When I apply to the school can I select to send only the highest score or all scores will be sent to the school.

Any time you send an official GMAT test report to a school, scores from ALL gmat exams taken on or before that date will be included in it.

Suppose I send GMAT scores to 5 different schools second time, will schools I sent the scores first time know about my second score or do I have to resend it? If yes do I have to pay money.

No, schools to which you sent report only at the first time, won't know about your second result. As such, you will have to pay an extra fee for sending additional reports from your second try.

I copied the following from the GMAT website:

If you repeat the test, any scores for tests you have taken in the past five years will still be reported to the graduate management programs you designate as score recipients. If you repeat the test and want to resend your scores to programs that previously received your scores, you must reselect the programs at the time you take the test or order an Additional Score Report after your exam with the appropriate fee. Any repeated GMAT® testing is subject to the GMAT® retest policy.

So if my scores go down first reported schools will not know about it. But I guess there is no getting around it. You have to enter it in application form. Or is this a loophole?

If your scores go down on the second attempt, I don't think the first reported schools will know. You will have to send them the score report again in order for them to see the new score.
