Yes, which can modify either a noun:
The house, which I love, is on fire.
Or a noun phrase:
The beautiful house on the hill, which I love, is on fire.
IT CANNOT, however, MODIFY A CLAUSE (a phrase that has a verb and noun in it).
(E) Aspirin irritates the stomach, which
"Aspirin irritates the stomach" is a clause, not a noun or a noun phrase. "Which" here can only be modifying "stomach." This makes little sense in light of the rest of the sentence.

I would choose (B).
(A) you can eliminate because "it" refers to either the aspirin or the stomach-- neither of which is an "avoidable" object. Similarly, in (D) it is unclear whether or not "it" refers to either the aspirin or the irritation. Assuming it does refer to the irritation, it's still awkward and wordy with the unnecessary "although".
(C) is wrong because the "fact" is also not an "avoidable" object.
(E) is incorrect, because "which" modifies the noun immediately preceding it-- "stomach" which is incorrect. Stomach is not an "avoidable" object.
