Hi,
Currently working as Assistant Manager (IC role) at a Fortune 15 BFSI. Won't be promoted to Manager in time for Fall'22.
Have an offer from a small startup-ish(16 year old) company in Travel industry as a Lead (matrix leadership).
Should I leave a Fortune 15 brand where I have 1 promotion in 3 years for this exciting role? How does it look on an MBA application if the latest role is 6 months old, in a startup but leadership?
Any guidance will be really helpful

Don't worry about BSchool too much. Your personal career growth and your bschool success should be aligned.
It is good that you are thinking about the optics. I think both could work and both would be about equal in their impact on the AdCom. Having a good strong brand name is super helpful and it is good you have it there. If you are excited about the position and you see it as growth, then absolutely take it... but keep in mind that LOR may be tricky so you want to do your best to keep bridges intact with your old boss so you could ask them for a second LOR since your new boss would probably not be able to write anything good about you in just 6 months

I am banking on the old boss for my LOR for all of my applications. Are there B-Schools that strictly ask for the current boss' LOR?
