IMO, D is correct.

Unlike 50 years ago, women in today’s world may choose to juggle a career and a family.

(A)Unlike 50 years ago, women in today’s world
Wrong. Compare "50 years ago" and "women".

(B)Unlike 50 years ago, today’s women
Wrong. Compare "50 years ago" and "women". Moreover, "today's women" sounds awkward.

(C)Women today, opposed to 50 years ago
Wrong. Compare "women" and "50 years ago"

(D)Unlike their counterparts 50 years ago, women today
IMO, Correct comparison: "women" vs "their counterparts" ==> make sense.

(E)Opposed to their counterparts 50 years ago, today’s women
Wrong. Change meaning. "oppose" differs from "unlike". Moreover, "today's women" sounds awkward.
