In A "but not with" isn't parallel to anything
it should say covered the entire cost of X, but not of Y
answer choice B incorrectly uses verb-ing modifier
So C

D and E - out from the start, as which refers to cost - but it is illogical.
A - of and "with" not ||
must be cost of hospitalization, but not of out patient services.
between B and C, in B ing modifier is illogical. Gvt has instituted a HCP to ensure smth, by making citizens to pay...
in C, for which correctly refers to out-patient services, so C looks good.
