The conjunction ‘if’ is used to convey a condition.
The conjunction ‘whether’ is used to introduce a single alternative, the other one being understood or implied. In this sentence, it is the latter meaning that is intended.
Options A and D contain the conjunction ‘if’. So, they can be eliminated.
In Option E, the pronoun ‘it’ is ambiguous; the antecedent of the pronoun is not clear. So, Option E can be eliminated.
Options B and C are identical except for the tense form.
Since the sentence states a general truth, the simple future tense – will stem – is inappropriate. Since Option C contains the simple future tense, it can be eliminated.
Option B has the appropriate conjunction ‘whether’ to convey an implied alternative; the alternative implied is that the problem may not stem from a malfunctioning of the inner ear and of the auditory nerve.
This option also contains the appropriate simple present tense form to convey a general truth.
