I’ll go with C
Here’s my explanation;

Extent here is used to emphasise the situation.

A: ... concerned about the extent that... Here it lacks emphasis, concerned about the extent. Extent that is okay but if it was to such an extent that it would be much better in terms of meaning.

B: who creates trouble here, although we want to refer to list of parents, educators and medical professionals, using who constraints only to medical professionals. Also a meaning error!

C: list is parallel and extent to which is correctly used to emphasise the situation. CORRECT

D: and should be used in list which has more than two items. Using verb-ing having is illogical here. Also, extent that does not shows emphasis (meaning error)

E: Although we are aware about the usage of dummy it. Here it creates a problem, as the rest of sentence is constructed in passive tone, the intended meaning is not conveyed.

Sentence Structure:
Because of the risks
-------associated with childhood obesity (CO)
------------------------—it (CO) can lead to diabetes, cancer, cardiovascular disease—
many parents,
educators, and
medical professionals (|| subject)
have become concerned (IC)
------about the extent (Limit)
------------that unhealthy foods are advertised to children (DC).

Inferred Meaning:
-Many parents, educators, and medical professionals have become concerned about X
- Here X is "limit to which unhealthy foods are advertised"
Error Analysis:
1. Sentence is not conveying the clear meaning
2. In the DC part "that" appears to be acting as Connector rather than a emphasis word
3. DC is already connected with IC by preposition "about" - therefore, can also say that sentence has structural error

A) many parents, educators, and medical professionals have become concerned about the extent that
- Error as highlighted above

B) many parents, educators, and medical professionals who have become concerned about the extent to which
- there is no verb for main clause(IC)

C) many parents, educators, and medical professionals have become concerned about the extent to which
- here "to which" is acting as emphasis word
- Conveying clear meaning
- CORRECT

D) many parents, educators, as well as medical professionals having become concerned about the extent that
- Parallelism Error ("as well as")
- Structural Error (use of "that")
- Missing Verb - "having become..........." acting as modifier

E) it has become the concern of many parents, educators, and medical professionals about the extent to which
- Pronoun ambiguity (usage of it)
- also this sentence appears to have modifier and meaning error - it is not clear the prepositional modifier "about the extent ..." is modifying which entity.

Because of the risks associated with childhood obesity—it can lead to diabetes, cancer, cardiovascular disease—many parents, educators, and medical professionals have become concerned about the extent that unhealthy foods are advertised to children.

B) many parents, educators, and medical professionals who have become concerned about the extent to which --> not an independent clause, it should be followed by IC

D) many parents, educators, as well as medical professionals having become concerned about the extent that --> same as B, as well as is unnecessary, or we need conjuction between parents and educators

E) it has become the concern of many parents, educators, and medical professionals about the extent to which --> making it more comlex, when we can directly say that " many parents, educators, and medical professionals have become concerned about"

Now, A vs. C

A) many parents, educators, and medical professionals have become concerned about the extent that -->

"that" is modifying the extent
"the extent unhealthy foods are advertised to children." --> two subjects
"unhealthy foods are advertised to children the extent ." --> structural error

C) many parents, educators, and medical professionals have become concerned about the extent to which -->

"to which" is modifying the extent
"unhealthy foods are advertised to the extent to children." --> makes more sense

Am I going in right direction to eliminate the option A? generis
Thanks! :)

Because of the risks associated with childhood obesity—it can lead to diabetes, cancer, cardiovascular disease—many parents, educators, and medical professionals have become concerned about the extent that unhealthy foods are advertised to children.

A) many parents, educators, and medical professionals have become concerned about the extent that -> "about the extent that" is incorrect.

B) many parents, educators, and medical professionals who have become concerned about the extent to which -> We don't have the main verb. Incorrect.

C) many parents, educators, and medical professionals have become concerned about the extent to which -> "To which" makes sense. Let's keep it.

D) many parents, educators, as well as medical professionals having become concerned about the extent that -> "as well as medical professionals" is unnecessary. Incorrect. We have C better option.

E) it has become the concern of many parents, educators, and medical professionals about the extent to which -> "It" doesn't have a precedent. Incorrect.

So, I think C. :)
