My answer is (A). It took me 1 minute.

(A) No ostensible error. "ever since" strongly suggests that a perfect tense should be used.

(B) "sudden" should definitely be "suddenly". "ever since" strongly suggests that a perfect tense should be used.

(C) "ever since" strongly suggests that a perfect tense should be used.

(D) See (B)

(E) "sudden" should definitely be "suddenly".

milanrajb , your analysis is mostly correct with one wrong turn.

As far as the analysis about the clause underlying the choice of "sudden," that analysis took a slightly wrong turn.

As I explained in the OE (linked below), appear functions as a linking verb similar to seems, in which we are given more information about the subject of the sentence.
The decision does not "show up suddenly."
The decision seems sudden (abrupt, unexpected).
We need the adjective sudden, not the adverb suddenly.
Only option E gets the verb tense and the adjective right.

You are correct that the answer is E.

You are also correct that "since" creates a need for a perfect tense to bridge past and present.

"Ever since" requires an even more specific verb: present perfect continuous. But you have exactly the right idea.
