Usually the last line of the passage depends on the line that immediately is preceding to it.
Here "The 14-member panel found disturbing evidence that interruptions in vaccination programs have allowed the virus to break through in some parts of the world." Talks about the disturbing evidence that interruptions in vaccination programmes have allowed...
So logically the consequences of this lapse would follow which are best represented by If the situation remains unchecked, it could result in the failure to eradicate one of the most serious vaccine preventable
diseases.
Hence Option (D) is our answer.

Thanks for your explanation. Based on what you said, why is E not the answer? The way I see it, What follows after the last sentence could very well be that the members of the committee were very disappointed in the findings.
