Sentence: The president ordered that Congress takes an action on the bill soon.
This is apparently correct in the grammar book. Isn't it supposed to be "ordered that Congress take"? Is "takes" a simple verb form?

You're right--the sentence should say "take." This is an example of the command subjunctive. In these cases, rather than varying the verb to match the subject, we always use the bare infinitive (infinitive minus "to") for the verb. A few examples:
I suggest that you arrive early.
I suggest that she arrive early.
I demand that my books be returned.
I demand that my money be refunded.
He commanded that the troops remain in place.
He commanded that the guard remain in place.
