4) What Schools are you planning to apply to?
If you took your 2nd Official GMAT at a Test Center, then you might also choose to purchase the Enhanced Score Report. While the ESR doesn’t provide a lot of information, there are usually a few data points that we can use to define what went wrong on Test Day (and what you should work on to score higher). If you purchase the ESR, then I'll be happy to analyze it for you.

Please see my responses below.
1) Oct 2021 GMAT Score - 590 (V23, Q48)
2) I studied for atleast 6-8 hours daily for over 4 months
3) I gave my mock tests mainly towards the end August and early September. Listing some of the mock tests scores below.
GMAT Official Practice Test 1: 650 (V28,Q49) (end August)
GMAT Official Practice Test 2: 720 (V38,Q50) (9th September)
LBS Mock test range 700-750
Princeton Mock test: 680 (V33, Q51) 27th August
Manhattan Mock Test :640 (V27, Q49) July End
4) I'm aiming for NYU Stern, UCLA, Michigan Ross, LBS, CMU, HEC, Imperial, NUS, Warwick, Duke, Chicago Booth, Cambridge etc
5) Unfortunately, I gave my both official GMAT attempts online
