IMO, if you want to enter those industries you should do LBS or Columbia, but even that might not do it. You should aim for a third industry, but if you want to go to NYC or London, nothing changes I guess.

If you want to do IB, I would seriously consider Stern, they are doing great over there.

Fallback would be consulting. For me it is now mostly between LBS and CBS J-Term. Any suggestions?

At risk of sounding pesimistic, I think your Fallback is more the Realistic Industry. Many (and I mean MANY), don't make it to PE, BB, etc.

But if consulting is an option, and probable your future industry, I think it comes to where you prefer to stay. NYC? Definitely Columbia. London? I'd look at that INSEAD offer with more attention. It simply places excellent in consulting, and it will open the door to about any company. It's been said that entering to MBB in London is nothing short of brutal, but you could land a more boutique firm wiht great clients.

INSEAD will get you to the same places than LBS in consulting in London, IMO it will get you further. And there the ratio cost/benefit is much better.

If consulting, I would simply go back to my current firm. Is getting into top-consulting firms more difficult than getting into BB?

INSEAD is basically out because of location and high consultant penetration. And I very much prefer London and NYC over Singapore/Fonty.

In London? It is pretty hard. BB would require some experience, that's what (I think) makes it hard, but I would ask someone who knows more of this industry.

Another consideration is international profile of the student body. Responding to the LBS vs. Columbia J-term question, for example:

Columbia has New York, which I think is a big plus, since New York's optimistic mindset will show you that anything really is possible. However, Columbia is still 2/3 American. That's great if you want to work in the US or want to be molded in an American cultural crucible (a very empowering experience for Asian women, for example).

At LBS, however, no one culture dominates, which is a great experience for e.g. Americans unaccustomed to working outside the US, or for those looking to land in a non-US environment post-MBA.

Expect your MBA experience to mold you considerably. You will not be quite the same person when you emerge. Make sure the environment corresponds to the experience you're looking to shape you during the course, and prepares you for the environment you hope to work in when you get back to the workplace.
