Can someone explain this question?

I am confused by the fact that Set must contain only distinct numbers......

To answer your doubt: yes, mathematically a set consists of only distinct elements. That's why real/official GMAT questions use word "list" or "data set" instead, which can contain same elements. This is not a perfect question in this sense.

Set S contains the elements {– 4, x, 0, 17, 1}. What is the median of set S?

(1) x > 1
(2) x < 2

S = {– 4, x, 0, 17, 1}
Lets try to arrange the digits.
-4, 0, 1, 17 ....... x

Option 1 : x>1, so arrangement becomes -4, 0,1 ,x ,17
So median = 1
SUFFICIENT

Option 2: x <2,
case 1: x = 1 -4,0 , 1, 1, 17 ...... median = 1
case 2: x =0 -4,0, 0, 1, 17 ........median = 0
case 3: -4=< x <0 -4, x, 0, 1, 17 .............median = 0
Case 4,: x <-4 , x, -4, 0, 1,17 .............median = 0
NOT SUFFICIENT
