The last line of first paragraph mentions "adverse selection" leading to market failure. The last line of second paragraph also mentions "market failure" stemming from "adverse selection". Thus, the author is identifying an alternative explanation that it disputes.

The author is trying to explain that what appear as greed and discimination on the part of the insurance companies are "necessary evils". Yes, they are evils but they are necessary.
