The key to handling complex sentences is to ignore the irrelevant bits - most commonly, the parenthetical (i.e. between commas) comments and long descriptive phrases beginning with prepositions.
Let's restate the original sentence, minus the colour commentary:

Perfect construction - choose (A)!
Even if all you recognize is that you need to begin with "that", you eliminate B, C and D. E has the awkward "if the present rate did continue", so we eliminate it and choose A.
The thing to note here: The second that in the sentence is part of the idiom so xxxxxx that and hence is indispensable. It will be easy to eliminate b, c and d on that count. In e, the conditional clause is using an untenable “if the present rate of infection did continue (a past), will become (a simple future) extinct within the next 20 years.” A is the answer. Plus we must eliminate the parentheticals that obscure the gist as skovinsky has rightly pointed out.
