extreme - adjective, Lethargy - Noun heightened - adjective, irritability - Noun therefore apparent confusion is parallel to the above two and the right choice
What is extreme & lethargy in this case ? Are they adverbs ? What about heightened and irritability?

extreme & heightened are adjectives modifying nouns lethargy and irritability, respectively. And so is apparent that's modifying confusion in D.
Adjectives give us an extra information about noun. Water is noun Hot water, cold water, dirty water, colored water Here hot, cold, dirty, colored are all adjectives.
Adverbs modify adjectives, verbs or other adverbs. extremely hot water here extremely is modifying the adjective hot as it is telling us the degree of hotness. Adverbs usually have a suffix -ly: seemingly, boldly, angrily.
