B&C are close.
Original sentence is lengthy as well as uses passive voice. B corrects both but idiomatically sounds wrong ("the patient" is used twice). C uses passive voice but is idiomatically correct.
I'll go with C

The question sounded like testing 'common sense' rather than grammar.
