Based on the recent decline in enrollment, the admissions office decided to reevaluate its recruitment strategies.
Does the modifier "based on the recent decline in enrollment" modify the noun office accurately? Or is this a case of misplaced modifier.

Yes it is a misplaced modifier issue.
It seems to indicate the Office is based on the recent decline in enrollment.
It should start with USING the records...

with misplaced modifiers.... immediatly after the comma the subject that is modfied must follow..
