This is the almost same as question 20 in OG11.

The only reason (B) doesn't qualify is that it uses pronoun 'it' with no clear reference according to OG11.

OG 11 has one this type of question. "Using the account of a survivor" is correct meaning.

Point to be noted:

INCORRECT: Based of my knowledge, I answered the question.
I don't base my answer on my knowledge. But I use my knowledge to answer the question.

CORRECT: Using my knowledge, I answered the question.

Based on an account from a survivor who talks about her own history of abuse in childhood and in adulthood, the author weaves a story about the experience of multiple selves, and its impact on other family members.

A. Based on an account from a survivor who talks on her own history of abuse in childhood and in adulthood
B. Basing it on an account from a survivor who talks about her own history of abuse in childhood and in adulthood
C. With an account from a survivor who talks about her own history of abuse both in childhood and in adulthood
D. By the account from a survivor who talks about her own history of abuse in childhood and in adulthood
E. Using the account of a survivor who talks about her own history of abuse in both childhood and adulthood

A-D: Misplaced modifier. All of them modify the story. Only E modifies the author.

Will go with E.

The author doesnot based his story only on the survivors account. Infact he uses the survivor's account to create a story.

Also. use of both is appropriate here.
